Submitting Post-Treatment Records

Once each of your Oliv treatments are complete, we want to see how your patient has progressed. You will have the opportunity to submit records of your post-treatment results and in return that treatment's retainer order is on us! Your treatment may even be featured on the Oliv website.

To submit your records at the end of each treatment, first take new, updated versions of the records that follow and bring them to the form linked here. 

We will need:

• The patient's name and Oliv treatment ID
• Photos, in the same format as taken at the start of treatment
• Digital scans, upper and lower
• A post-treatment X-ray

Use that form to enter the required ID info and then to upload all records. Please note where multiple uploads or single uploads might be needed or allowed. Once that's complete, the last question is all about your free retainer! If you have already submitted these records for a new retainer treatment, we will cover the cost of that on your next bill; but if you haven't yet, don't forget to submit right away to take advantage of the offer.
